1. We need the Shipping Address of the school receiving this table.
2. Does the Shipping Address have a loading dock that can accept tractor trailer deliveries?
3. Does your table need to be equipped with casters for portability (recommended)?
4. Does your table need to be equipped with safety rails for fall protection (recommended)?
5. Do you have any students who require transfers to the Changing Table via a patient lift?
6. We require up to 10 weeks lead time for the table build. Is this acceptable?
